School Directory Registration
REGISTRATION SUBMISSION DEADLINE: SEPTEMBER 13, 2018

Entering your family information in this section is necessary to be in the directory, even if you have done it in prior years.

Please fill in the appropriate fields as completely as possible and use upper and lower case (not all UPPER or all lower) when you fill out the forms. Your prompt cooperation is appreciated.

This directory will be published online in a password protected page.

The school directory is for the use of school families only and will have three sections: family information, a business directory and a babysitting directory.  Please fill in the form as completely as possible and note there must be at least one parent name field filled in for the school directory. Please use the "NOTE" box at the bottom of the form to let us know of any special requests or to clarify any of your entries.  Only use the name your child was registered under to avoid any confusion.  Please check your form for errors before submitting and be sure to register only once.  If you have any questions, please contact Mrs. Lisa Gallina at: [email protected]
